I just tested again to verify what I said. Indeed you don't have to re-do all the groups in your course but all groups where members change. (In case you let the students choose their groups through a plugin like groupchoice this still means you have to do all the groups again, I guess.)
So, if S1 ist part of Group A for an assignment and wants to join Group B for the next assignment, then the teacher has to create a new Group C with S1 and the members of Group B. And he has to create a new group D for the remaining members of Group A. Because for the new grouping you cannot re-use the old group A (and neither you can re-use Group B). Another Group E that remains stable from the first to the next assignment can remain as is but has to become part of the new grouping of course. The groupings in this scenario are Grouping 1 (Groups A, B, E) for Assignment 1 and Grouping 2 (C, D, E) for Assignment 2.
So, basically it works with groupings but still to me it seems rather complicated and difficult to explain to users.

That's the error that I get when I use Moodle's built-in 'Test Ghostscript Path' link in Admin, Plugins, Activity Modules, Assignment, Feedback Plugins, Annotate PDF.
My host has told me that Ghostscript has been installed and I get the green tick when I set its path.
When I test the Google OAuth2 converter, everything is fine and I can see the lorem ipsum document. I would have thought that OAuth2 would pick up the grading task but something seems wrong.
I have set OAuth2 to be the only document converter (I don't think I have LibreOffice or unoconv on the server).
I use groups a lot (actually for a way different purpose but that besides the point). I tell my teachers that Moodle gives you the option to create groups for assignments - because students are not necessarily in the same group for each assignment, they should create groups with a naming convention something like this: Assign1-GroupA, Assign1-GroupB, Assign2-GroupA, Assign2-GroupB and then place them in Groupings named Assign1, Assign2. Does it make it easier no, but it helps them to understand how it works...that the grouping is specific to the assignment and the group naming makes it easier to understand and quickly find when they are looking at a long list of groups...it also helps it make sense for the students who do not know about groupings but will hopefully now understand why they are in different groups...
I'm looking at the weirdest case that I have ever seen in over 10 years of Moodle usage.
Somehow, a turned in assignment, has switched from user A to user B.
Has anyone ever experienced this? It causes a BIG problem internally as from now on, in the entire team, the discussion has burst out about the safety of Moodle. Some of the assignments contain very sensitive information (like medical data) and when it ends up under the wrong user we are in BIG trouble.
I don't have more evidence than the specific log files of this course, which clearly show user B uploading a file at time X. And user A having a turned in assignment at exactly the same time, but no log files proving that user A turned in anything.
What are the odds of mixing sessions in Moodle? This is even a single server with a single database (no clustering). I'm puzzled, have no answer for this... and that's exactly what the team is not fine with. They want to know HOW and WHY this happened.
What permissions does User B have? Is it possible they logged in as user A using the Login As feature?

Another possible cause that you might investigate is if the assignment was set for group submission and then switch back from group submission after some students submitted.

This is not possible. The logs would show 'User A as User B'. I can see myself in the logs that way if I log in as another user.
Also, both user A and user B have student rights... they should not have the ability to log in as another user.
Eureka. I think you nailed it. The group setting on the assignment is set.
However, that does leave me puzzling a bit. I understand now why an submission made by user A ends up with user B. I do however not get why it isn't also with User A.
The option 'require group to make a submission' is set to NO. So if User A was in a group at the time of submission, it would end up with User B if they share a group.
What if User A is later removed from that group, would the assignment just stick to user B and not to User A?
Yes, because you do not have the requirement for all users to submit so the one submission is attached to all existing group members. Nice catch Spall!

Moodle noobie here. We are getting reports from our students that they are unable to submit assignments. When I log in as a student, and click on the assignment and select "add Submission", all I get are the "Save changes" and "Cancel" buttons (see screenshot). The actual object to drag the files onto or to bring up the file picker doesn't show.
When I log in as the administrator and go to edit the course, the file upload box shows as expected.
I've tried changing the YUI settings as mentioned in other (older) posts, and I've tried different browsers. I've verified that they have the correct permissions to submit assignments.
We're running Moodle 3.1.13 on IIS and MS SQL.

For security reasons, if you are using the 'login as' feature in Moodle, then you cannot upload assignments on behalf of a student. The only way to submit an assignment is to login with the username + password of the student.

Interestingly, it is possible to hide on Gradebook however, the grades for attempts are still visible on assignment page (Please see screenshot below).

In the scorm settings, I think if you hide the summary page, that will stop them seeing this page. Those attempts are coming from the scorm package - you might see if you can hide them in your scorm authoring software..
