How to define rubric with scale

I want to define rubric as shown below:

CATEGORY

Level 1- 52%; 1 55%; 1+ 58%

Level 2- 62%; 2 65%; 2+ 68%

Level 3- 72%; 3 75%;  3+ 78%

Level 4- 84%;  4 88%;4+ 95%;  4++ 100%

Knowledge
