We are facing the same problem since we upgrade our production installation 2.9.2 to 2.9.3+ (20151204) 10 days ago. Since then, we upgrade to version 20151209 but this doesn't change anything. We searched forums for a solution but at this time, it seems that only one russian forum talked about the problem without solution at this time (https://moodle.org/mod/forum/discuss.php?d=324024&parent=1301326). Since then, there is another post https://moodle.org/mod/forum/discuss.php?d=324585talking about the problem but with no solution yet.
The problem occurs at every attempt in any course.
The only solution we found for now is to activate fast correction to give grades to our students. This information is kept by the system.
Maybe someone has tried to soluce the problem with installation of version 3.0 ?
Any help or idea would be appreciated.
