Standard Feedback form for Marking

Hi All,

I want to embed a standard form as part of the Gradebook marking process within the Feedback section. We need to standardize feedback for completeness and quality purposes. Instead of displaying a freeform text area for feedback I want to embed an HTML form to be completed.

Has anyone resolved this? I want to stay as vanilla as possible so editing the source is not ideal from my view.
