I have the same issue to Aarif where every time the students uploading their assignments, the teachers haven't received email notification alerts. I've checked maillog files and can't find specific logs that moodle is sending emails. Although it is inconsistent problem because some times email notification is working and other times is not. It seems the issue is the Moodle where I used version 2.2.1 and Im not sure if this will fix when upgrading to the latest moodle version.
I would love to hear and appreciate to anyone who have any ideas and experiences to rectify this issue.
